Yes - this optional form is to be used by a vet collecting the sample and confirming the identity of the dog by a permanent identifier (microchip or tattoo), for an additional level of assurance that the sample was collected from the specified dog.
This form must be signed by a licensed vet and once completed, it can be uploaded to the “Documents” section of the dog’s profile in the form of a PDF or photo.

How to add the form to your dog's profile:
- Scan a copy of the completed form.
- Upload it to your dog's Documents section. For instructions, please see: How to add documents to my dog's profile? Please note: If you would like to have this form accessible from your dog's public profile, it will be need to set to Public.
Please note: A microchip number needs to be entered within your dog's profile to show on their OFA Submission and other Embark results reports. This is true even if you have recorded a microchip number on the Veterinary Verification form.
